California Physician Offices


HSAG of California works with select groups of local physician offices with certified electronic health records to improve the quality and frequency of preventive health care services in order to optimize Medicare beneficiary quality of life and health care efficiencies.


HSAG of California provides assistance based on practice needs, which includes:

• Help submitting clinical quality measures (CQMs) data directly from Physician Quality Reporting System (PQRS)-certified EHRs to CMS to receive incentive payments.
• Identifying specific data elements for each preventive care measure.
• Establishing workflows to allow efficient and consistent data capture.
• Developing quarterly reports that allow timely monitoring of PQRS measures.
• Implementing a continuous quality improvement program to improve performance rates.
• Providing a Learning and Action Network (LAN) for information dissemination and online discussion with other providers in the state.

HSAG of California works with physician offices to improve care management processes and submit PQRS claims directly from EHRs to provide rate information for the following areas:

• Colorectal Cancer (CRC) Screening
• Breast Cancer Screening
• Influenza and Pneumococcal Immunization
• Low Dose Aspirin Therapy for Ischemic Vascular Disease
• Blood Pressure Control for Ischemic Vascular Disease
• Tobacco Cessation Interventions
