CA Nursing Home Clinical Project Manager

POSITION SUMMARY

Under general supervision, assists the Program Director in overseeing day-to-day operations of the Quality Improvement Organization (QIO) contract, which is designed to make care safer, promote effective coordination of care, promote preventive care, help communities support better health, reduce costs of care through continual improvement, and promote person- and family-centered care. This position is responsible for coordinating and implementing a broad range of collaborative and community activities among private and public entities to improve health and health care for all Medicare beneficiaries. Clinical topics include healthcare-acquired infections/conditions, pressure ulcers and physical restraints, adverse drug events, care transitions, and preventive services. Clinical settings include, but are not limited to, hospitals, nursing homes, home health agencies, physician offices, outpatient clinics and pharmacies. This position is also responsible for coordinating project activities including planning, budgeting, preparing and finalizing reports, and meeting project timelines and deliverables.

ESSENTIAL FUNCTIONS

  1. Coordinates project management tasks including planning, budget analysis, staffing, communication (written and oral), and training.
  2. Communicates/consults with health care providers and communities on a regular basis to assist with advancing their quality improvement objectives and provides technical assistance and recommendations as needed.
  3. Assesses current health care environment, provider and community processes, support systems, and barriers. Based upon assessment findings—and in collaboration with providers, partners, and stakeholders—recommends appropriate interventions.
  4. Communicates and works well on tasks with ancillary and ad-hoc team members, community partners, and stakeholders to plan, facilitate, and execute activities.
  5. Utilizes clinical expertise, problem-solving techniques, flow diagrams, statistical process control charts, cause-and-effect diagrams, force field analysis, root cause analysis, and other quality improvement methods to demonstrate to collaborative and community participants how to reengineer existing processes or design new ones to improve performance.
  6. Provides necessary education and materials and directs providers to resources that will assist them with understanding and utilizing problem-solving and quality-improvement techniques.
  7. Assists in the development of training programs and the provision of technical assistance to providers, partners, stakeholders, and community members, as well as participates in educational presentations as required.
  8. Possesses the ability to discuss complex health care issues during face-to-face meetings, teleconferences, and through written correspondence with providers and stakeholders. Refers questions to appropriate HSAG associates and Centers for Medicare & Medicaid Services (CMS) contractors as needed.
  9. Remains current and knowledgeable about new initiatives, activities, opportunities, tools, and techniques through research, training, education, and a variety of nationally recognized sources.
  10. Routinely documents and reports on project status, on-site visit activities, provider activities, and barriers/solutions using appropriate HSAG and/or CMS reporting tools and databases.
  11. Develops written reports, proposals, and project budgets. Assures contract tasks meet customer and budget requirements.
ACCOUNTABILITY

Works under the general supervision of the Program Director. Accountable for the timeliness, completeness, accuracy, and quality of all assigned functions and tasks.

WORK CONDITIONS

In this position, the employee will be subject to environmental conditions related to vehicular travel and possibly air travel to and from locations—which may include overnight stays. Additionally, the employee must also be willing to work in a cubicle office environment, use a computer, and attend meetings when not traveling.

  8. Use a computer and appropriate software programs.
  9. Adhere to established company, state, and federal confidentiality policies.
  10. Tolerate and accept opposing views.
  11. Participate in and contribute to project team meetings.
  12. Adapt to different situations and remain flexible in a fast-paced, changing environment.
  13. Cope with stressful situations by maintaining high performance when confronted with pressures of internal and external deadlines and time limits.
QUALIFICATIONS

  1. Candidate must be an experienced health care professional with a bachelor's degree or higher in appropriate science or related discipline and hold a current California healthcare-related license (RN, NHA, NP, PA, PharmD, etc.)
  2. Three or more years of health care experience in a skilled nursing facility environment.
  3. Knowledge of and experience with root cause analysis, quality improvement tools and techniques, customer service, and culture-change concepts.
  4. Clinical experience specific to infection control, pressure ulcer reduction and/or physical restraint management, and drug safety is preferred.
  5. Successful experience with meeting planning, group facilitation, coalition building, and establishment of community-focused learning and action networks.
  6. Knowledge of survey standards and requirements for The Joint Commission, California Department of Public Health, Title 22, and CMS is preferred.
  7. Possess general understanding of the changing health care environment.
  8. Proficient in Microsoft Office.
  9. Proficient in oral and written communication and interpersonal skills.
  10. Grant writing skills a plus.
