Partners in Care Foundation (Partners) is providing no-cost, chronic disease self-management program (CDSMP) workshops for older adults, 60 years and older, living in Los Angeles County. These programs have proven to help older adults better manage their chronic conditions, improve their quality of life, and lower healthcare costs.
To register for this 6-week evidence-based workshop, email cwreferral@picf.org and include, at a minimum, the patient's name, and telephone number.
