The Area Agency on Aging (AAA), Region One, provides Diabetes Empowerment Education Program™ (DEEP™) workshops at no cost to Maricopa County residents 60 years of age and older. A DEEP workshop could significantly reduce costly hospital readmissions, emergency department visits, and nursing home placements.
The workshops are offered in-person and online. To refer your patients, 60 years and older to these six-week, evidence-based workshops, fax 602.230.9132 (Attention: Health Promotion) and include, at a minimum, the patient’s name and telephone number.
