The Area Agency on Aging (AAA), Region One provides Diabetes Empowerment Education Program (DEEP) workshops at no cost to Maricopa County residents 60 years of age and older. A DEEP workshop could significantly reduce costly hospital readmissions, emergency department visits, and nursing home placements.
The workshops are offered in person and online. To refer your patients, 60 years and older to these six-week, evidence-based workshops, fax 602.230.9132 (Attention: Health Promotion) and include, at a minimum, the patient’s name and telephone number.
