HSAG Recognizes Nursing Home Quality Improvement Efforts

As part of an initiative to distinguish nursing home quality improvement efforts throughout the state of Arizona, Health Services Advisory Group (HSAG), Arizona’s Quality Improvement Organization, has developed the Nursing Home Quality Initiative (NHQI) Recognition Program. Recently through this program, participants received recognition for their quality improvement efforts during the 7th Scope of Work (SOW) from the second quarter (Q2) of 2002 to Q2 2004.

The table below notes the criteria for earning the different levels of recognition. Under each category is the method of distribution.

We at HSAG are proud to present Arizona nursing homes that met the Certificate of Achievement and Leadership criteria. They are:

From Q2 2002 to Q2 2004, participated in three HSAG-sponsored NHQI learning sessions/teleconference calls.
NHQI Achiever
-Certificate of Achievement

-Publication in NHQI Quality     Counts Newsletter*
-Mention on www.hsag.com*
above criteria, plus:
- Reached a relative improvement rate of 50 percent or better from Q2 2002 to Q2 2004 in one of the three HSAG focus Quality Measures (Pressure Ulcers, Chronic Care and Post Acute Care Pain) as reported on Nursing Home Compare.

-HSAG notification to the Arizona Health Care Association, Arizona Association of Homes and Housing for the Aging, Covenant Health Network, Arizona Health Care Cost Containment System (and health plans), Arizona Department of Health Services, and Medicare Advantage plan(s)*

- Achieved an Excellent Quality Rating on two most recent Arizona Department of Health Services annual surveys.
