The Alzheimer's Association offers information on all stages of dementia caregiving with tools and resources to access on demand, including a helpline.

This is a non-profit, patient-driven organization, affording members opportunities to learn about people with similar experiences. In addition, they provide education on kidney disease, tips on how to slow its progression, and where to turn for help. AAKP will provide the answers and guidance you need to live a full and productive life.

This is a leading cancer fighting organization, engaged in funding research and patient advocacy.

The American Heart Association created Heart360®, a tool to help people understand and track the factors that affect heart health, including blood pressure, physical activity, cholesterol, glucose, weight, and medications.

The American Heart Association and the American Stroke Association developed a high blood pressure risk calculator which estimates your blood pressure category, and provides a view of your risk for heart attack, stroke, kidney disease, and more.

Diabetes Social Media Advocacy

Diabetes Social Media Advocacy (DSMA) is involved with and promotes social media in all its forms to empower people affected by diabetes and to connect them with each other to foster support and education. Among its offerings are two radio shows (see http://diabetescaf.org/our-programs-2/dsma/dsma-live-programs/) and a weekly tweetchat on Twitter (Wednesday nights at 6 p.m. PT; use the hashtag #DSMA).
