The study
Improving Hypertension Management in Primary Care Practice with the AAFP METRIC Program will evaluate the effectiveness of the American Academy of Family Physicians (AAFP) Measuring, Evaluating, and Translating Research Into Care (METRIC) Hypertension module in improving clinical outcomes in the management of hypertension related to blood pressure (BP) control.
This is a physician cohort study using a pre-post intervention design with a six-month intervention period. The study includes 42 physicians from practices located in Maricopa and Pinal counties in Arizona. Nine hundred twenty four (924) medical records will be randomly selected and reviewed including 11 patients per provider for baseline measurement and 11 patients per provider for remeasurement. The same patients may or may not be included in both sampling periods. The baseline measurement period will be the three months immediately prior to the start of the intervention period and the remeasurement period will be the three months immediately following the end of the intervention period.
At the start of the intervention period, physicians will receive national comparative feedback on their baseline-period clinical management of hypertensive patients according to the AAFP METRIC Hypertension Module quality indicators, which are based on primary care relevant measures from the American Medical Association (AMA)-convened Physician Consortium for Performance Improvement?. Based on the comparative feedback and the results of a self assessment of their individual office practices, physicians will select from interventions provided by the AAFP METRIC Hypertension Module to develop a quality improvement action plan. They will implement the action plan for six months using tips, templates, and resources from the AAFP METRIC Hypertension Module. Pre- and post-intervention data will be analyzed to determine if the group of physicians has improved in the clinical management of hypertension after the physicians have participated in the AAFP METRIC Hypertension Module.
