Network 13 Goals
To assist the Centers for Medicare & Medicaid Services (CMS) in meeting goals and in keeping with sound medical practice, the strategic goals of the End Stage Renal Disease (ESRD) Network Program are to improve:
- The quality and safety of dialysis related services provided for individuals with ESRD
- The independence, quality of life, and rehabilitation (to the extent possible) of individuals with ESRD through transplantation, use of self-care modalities (e.g., peritoneal dialysis, home hemodialysis), in-center self-care, as medically appropriate, through the end of life
- Patient perception of care and experience of care, and resolve patient's complaints and grievances
- Collaboration with providers to ensure achievement of the goals through the most efficient and effective means possible, with recognition of the differences among providers (e.g., independent, hospital-based, member of a group, affiliate of an organization, etc.) and the associated possibilities/capabilities
- The collection, reliability, timeliness, and use of data to measure processes of care and outcomes; maintain Patient Registry; and to support the ESRD Network Program
About the ESRD Network Program
Under the direction of CMS, the End Stage Renal Disease (ESRD) Network Program consists of a national network of 18 ESRD Networks, responsible for each U.S. state, territory, and the District of Columbia. ESRD Networks service geographic areas based on the number and concentration of ESRD beneficiaries.

This report details the needs assessment results from 2,835 patient responses and 374 provider responses to the needs assessments that were distributed in November 2018, representing the Network 13 service area. It includes analyses and information about issues associated with:
- The Dialysis Facility Compare (DFC) website
- Filing a grievance and the option of filing anonymously
- Involuntary discharge (IVD)
- Transitions of care
- Resources for coping with ESRD
- Permanent access options and barriers
- Infection control practices
- Pain management issues
- ESRD treatment modalities, including home dialysis and transplant
- Patient and family engagement practices
Providers can review and use this information in conjunction with their individual data when addressing delivery of ESRD care.

Successfully achieving this objective requires that professionals from within the healthcare and business communities play a key role on our boards and committees. The Network is looking for potential candidates who have demonstrated a commitment to or an interest in improving the quality and experience of care for people dealing with the complex chronic healthcare disease known as ESRD to participate on the Network's Board of Directors (BOD), Medical Review Board (MRB), Patient Advisory Committee (PAC), and Network Council (NC).
