The In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Survey is a standardized questionnaire that asks adults with end stage renal disease about their experiences at their hemodialysis facility. As part of the quality improvement project (QIP), facilities that serve 30 or more adult, in-center patients are required to contract with a CMS-approved vendor to conduct the CAHPS survey annually.
As of 2015, the ICH CAHPS Survey must be completed TWICE a year:
- Once in the Spring
- Once in the Fall
Both forms may be found on the ICH CAHPS website.
