How to Use the Pre-Admission Planning Checklist to Support Family Engagement and Reduce Preventable Hospital Readmissions
This week’s Patient and Family Engagement monthly learning event, entitled “PFE Metric 1 (Preadmission Planning Checklist): How to Use the Checklist to Support Patient and Family Engagement,” was designed to help HIINs, hospitals, and caregivers understand how to use several tools, including a pre-admission planning checklist and Caregiver Advise, Record, Enable (CARE) Act, to invite and support caregiver engagement at the bedside to potentially reduce preventable readmissions.
Dr. Thomas Workman, the PFE Contactor Senior Technical Advisor, chaired the event and was joined by Marcus Escobedo, Senior Program Officer & Communications Director, John A. Hartford Foundation, as well as a patient and family advisor.
