PFE Think Tanks are a monthly virtual meeting facilitated by Libby Hoy in which all HSAG HIIN hospitals are invited to learn and share best practices surrounding patient and family engagement. Participants of the Think Tanks will also be provided technical assistance to address specific issues, questions, or concerns regarding effective methods to engage patients and their families in healthcare.

How is your performance on CMS PFE metrics? Has your hospital worked on patient-and-family-centeredness but would like to achieve stronger results?

Facilitator Libby Hoy of PFCCpartners will lead peer-to-peer learning and exchange while providing expert technical assistance to help the hospitals meet PFE metrics. There is no cost to attend, and this event is open to all hospitals participating in HSAG HIIN.

Subsequent Think Tanks will be held on the fourth Tuesday of each month, from 12 Noon to 1 p.m. PT, starting on January 23 and concluding on September 25, 2018. 

Engaging Patient Family Advisors (PFAs) to Reduce Readmissions

Always wondered if PFAs can help you reduce hospital readmissions? The answer is yes! At this PFE Think Tank you will learn key strategies and practical tips to engage PFAs in reducing readmissions at your hospital. Hear examples from advisors informing the redesign of discharge processes to increase the confidence of patients discharged; and contribute your own hospital's effors to partner with patients and families to reach your readmission goals.
