Avoidable readmissions are frequently a result of ineffective communication with our patients during discharge planning. A strategy for improvement in this area is the use of teach-back during patient and family education. Join HSAG HIIN to gain knowledge and skills to implement an effective teach-back strategy.
- Identify methods used to effectively engage patients and families in the discharge planning discussion.
- Describe the assessment of patient’s understanding of care plan elements.
- Identify potential areas of misunderstanding between patients and caregivers during discharge planning discussions, and the use of the teach-back method to facilitate understanding.
