Summary from 10/18 Pacing Event: “Readmissions: Improving Care for Multi-Visit Patients”
This Pacing Event focused on a strategy for addressing hospital readmissions by presenting the “MVP Method” of improving care for multi-visit patients (MVPs). The MVP Method has been implemented to improve care for high utilizers of the ED and the inpatient settings, and for special populations, such as patients with COPD, sickle cell, homelessness, and opiate use disorder. The MVP Method has been successfully implemented by nearly 100 teams - from small, rural hospitals to urban medical centers.
The session featured a presentation by the developer of the MVP Method, Amy Boutwell, MD, MPP (Collaborative Healthcare Strategies), as well as a panel of MVP Implementation Faculty, including:
- Tim Stroupe, MD, MS, FAPA, from NYU-Langone Hospital;
- Diane Nanno, MS, CNS, RN, CCTM, from Upstate University Hospital;
- Geoffrey Peck, from Nathan Littauer Hospital; and
