This one-hour webinar will feature speakers from the Coalition for Compassionate Care of California on the topic of advance care planning and end-of-life care, as well as POLST implementation in California.
Objectives
- Learn about strategies to improve and enhance advance care planning and end-of-life care.
- Determine gaps in advance care delivery and where enhancements can be made within the hospital and community setting.
- Utilize and implement key processes to ensure the POLST form is used effectively to support end-of-life care.
