Health Services Advisory Group (HSAG) has great community building activities occurring in the East Valley for providers who are eager to work collaboratively to reduce avoidable readmissions, medication errors, and adverse drug events. At this kick-off event, we will review population data, identify areas of focus, and explore possible interventions that will improve the health and outcomes for beneficiaries in this community.
Who should attend: East Valley hospitals, nursing homes, home health agencies, pharmacies, community-based organizations, and providers.
Hospital CEOs: A table will be provided for you to bring three to four staff members who are the key to successful patient care coordination activities and hospital transitions in your organization.
Suggested staff members include: Chief medical officers; chief nursing officers; executive directors; pharmacists; heart failure coordinators; and directors of nursing, case management, emergency department, and auality
Post-acute providers and community organizations: Be part of this dynamic coalition as we develop interventions across all care settings.
