  • Examine the use of forms—Change in Condition and Situation Background Assessment Recommendation (SBAR)—that will increase caregiver confidence when assessing older adults in post-acute settings.
  • Identify processes to recognize symptoms of diabetes and hypoglycemia for older adults in post-acute settings.
