East Valley Care Coordination Coalition (EVCCC)—Quarterly Community Meeting
1. Review quarterly readmission data and progress being made in the community to conquer the “Perfect 10” challenge.
2. Hear about the progress being made in the various workgroups including project scope, interventions and metrics under development.
3. Profile best practices or promising practices occurring in this Community.
There is NO charge to attend however, registration is required so we can provide an accurate count for room accommodations.
