Social determinants of health are the conditions in which people are born, grow, live, work, and age. These circumstances are shaped by the distribution of money, power, and resources at global, national, and local levels. The social determinants of health are mostly responsible for health inequities— the unfair and avoidable differences in health status seen within and between countries. This webinar will describe the concept of social determinants of health, the role of social determinants in patient-centered medical homes (PCMHs), and how PCMH practices can work to improve the health of their patients by addressing social determinants of health.
This webinar is part of a series of quarterly patient engagement webinars hosted by the Ohio Patient-Centered Primary Care Collaborative (OPCPCC) Center for Provider and Patient Engagement in partnership with Health Services Advisory Group (HSAG). The webinar will begin with a half-hour presentation at 12:15 p.m. ET and will conclude with a 15-minute Q&A session. David Norris, of the Kirwan Institute for the Study of Race and Ethnicity will serve as the presenter.
