CMS Opens Medicare Evidence-Based Diabetes Prevention Services Model to Community Healthcare Organizations

Community organizations are now invited by the Centers for Medicare and Medicaid Services (CMS) to become service suppliers for the Medicare diabetes prevention program model. This program includes behavior change intervention programs designed for Medicare beneficiaries with prediabetes.
