Medical Worker Nurse Talking with Senior Patient

New York City Health + Hospitals (NYC H + H) deployed its electronic health record system in 2013, which included secure direct messaging, patient health data access, and prescription refills. The transition to this system is not yet done, as the provider determined that patient engagement was missing. So far, over 14,000 patients have signed up for the portal, but given the diversity and vulnerability of some of its patient pool, the provider had to do more to achieve broader adoption. This article explores the provider’s campaign, including tailored patient education to help close the gap. The system now includes patient portal liaison who communicate the portal’s benefits to patients and help with sign up. Read more about it at the Patient Engagement HIT article.
