Welcome to the California Nursing Home Quality Care Collaborative (NHQCC)!

Providing the highest quality care to nursing home residents is a mission shared by all nursing home professionals. The work done in each nursing home—from preventing pressure ulcers to appropriately using antipsychotic drugs—has a profound effect on improving the lives of California’s 105,000 nursing home residents.

With the California NHQCC, Health Services Advisory Group will work with nursing homes to identify opportunities for improvement using a data-driven, proactive approach to improving quality.

About the Collaborative

The collaborative aligns national nursing home quality initiatives and partnerships, such as the National Nursing Home Quality Improvement Campaign, the Centers for Medicare & Medicaid Services (CMS) Partnership to Improve Dementia Care in Nursing Homes, and Quality Assurance & Performance Improvement (QAPI) to improve quality of life and care for residents. Nursing homes participating in the collaborative will be working on clinical and organizational topics including:

  • Reducing healthcare-acquired conditions to achieve a Nursing Home
    Composite Score of 6.0 or lower by 2019.
  • Reducing antipsychotic medication rates.
  • Reducing avoidable hospital readmissions.
  • Participating in QAPI initiatives.
  • Increasing influenza and pneumococcal vaccination rates.
  • Reducing staff turnover and implementing consistent assignment.
  • Improving rates of resident mobility.

A Focus on Clostridium Difficile Infection (CDI) Reporting and Reduction

Learn more about the National Nursing Home Quality Care Collaborative (NNHQCC) Clostridium difficile Initiative and work with HSAG and the Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN) on a Centers for Medicare and Medicaid Services (CMS) Quality Assurance & Performance Improvement (QAPI) Initiative to prevent and reduce C. difficile in your nursing home.
