Biographies: Mr. Nelson is a former Kidney Transplant Affinity Group Subject Matter Expert. Mr. Bailey is the Associate Director of Patient and Family Engagement for the ESRD NCC. Kim Buettner is the Associate Director of Patient and Family Engagement for the ESRD NCC.

  • Identified the role of the NPFE-LAN in fostering patient engagement to develop education resources.
  • Discussed resources created by the NPFE-LAN Transplant Affinity Group and share methods for spreading the resources to patients.

Presenters: Robin Taylor, MSW, LSW

Biographies: Robin Taylor is a renal social worker and Kidney Smart Modality Educator for the DaVita Hackensack Dialysis facility in Maywood, New Jersey, where she has worked for the past twenty five years.

Presentation Objectives:

Ms. Taylor shared how her facility uses a multi-disciplinary approach to educate the patients and staff about kidney transplant. During the presentation, she:

  • Described different approaches used to educate staff about transplant.
  • Discussed ways to increase transplant awareness for the in-center patients.

Transplant QIA LAN Identified Best Practices 2019

The following best practices were identified from the Transplant QIA LAN events in 2019:

  • Educate patients at their level of readiness and re-educate existing patients who may not have been interested when educated in the past.
  • While educating them, listen to why patient are not interested in being referred for a kidney transplant and talk through their concerns and possible misconceptions.
  • List eligible patients at more than one transplant center.
  • Provide education regarding the steps to transplant, including details of what to expect at each step.
  • Use a tracker to monitor patients as they move through the transplant referral and work-up process.
  • Incorporate fun activities around patient education.
  • Host a lobby day that includes peer support and mentorship from a previously transplanted patient.

Identified during the October 16, 2018 Transplant QIA LAN:

  • Follow-up with patients after the referral has been made.
  • Ask transplant centers for a patient report/updated list of patient status.
  • Ensure that transplant education is accessible in a variety of formats.
  • Encourage living donation.

Identified during the August 21, 2018 Transplant QIA LAN:

  • Listing patients in other states that may have a shorter wait time.
  • Following-up with patients after the referral has been made.

Identified during the June 19, 2018 Transplant QIA LAN:

  • Develop a transplant interest form to review with patients.
  • Refer patients to another transplant center if they do not meet the criteria for the first.

Identified during the April 17, 2018 Transplant QIA LAN:

  • Identify a Peer Mentor/Patient Navigator who could provide support and assistance to patients as they go through the transplant waitlist process.  

Identified during the February 20, 2018 Transplant QIA LAN:

  • Creating a transplant binder to track the 7 steps.
  • Developing a communication and tracking process between the facility and the transplant center to update important information regarding patient demographics, insurance, medical condition, etc.
