Integrating Diabetes Self-Management Education in Your Community Health Centers

Health Services Advisory Group (HSAG) is available to assist community health centers in helping patients prevent and better control diabetes. As the Quality Improvement Organization for your state, contracted by Medicare, we have a team of licensed diabetes trainers available to conduct two-hour diabetes self-management education (DSME) classes spanning six weeks among your Medicare beneficiary patient population—all at no cost. HSAG also is available to train your staff to conduct DSME for all of your patients.

Objectives: At the completion of this webinar, participants will be able to:

  1. Describe the Everyone with Diabetes Counts (EDC) program.
  2. Identify overlaps with current criteria for Health Resources and Services Administration's (HRSA’s) Diabetes Quality Improvement Initiative and Patient Centered Medical Home (PCMH).
  3. Recognize successes and challenges from Health Centers currently providing DSME. 
  4. Evaluate post-workshop survey results from EDC graduates.
