Objectives
- Convene San Francisco, nursing homes, skilled nursing facilities, and home health agencies to engage in conversation and action to reduce readmissions through improving care transitions.
- Explore the importance of advance care planning and best practice interventions.
- Determine readmission challenges to improve care coordination and patient outcomes.
- Discover how to overcome barriers to designing and implementing readmission reduction strategies.
Who Should Attend?
San Francisco Care Coordination Collaborative Meeting, NHAs, DONs, Directors of Quality, Directors of Case Management, Home Health Administrators, Nurses, Medical Directors, Physicians, readmission leads, and anyone interested in reducing readmissions.
Day of event: registration and breakfast are from 8:30 a.m. to 9:00 a.m. and the event starts at 9:00 a.m.
