The County of San Diego Health and Human Services Agency is sponsoring no-cost, chronic disease self-management program (CDSMP) workshops for older adults, 60 years and older, living in San Diego County. These programs have proven to help older adults better manage their chronic conditions, improve their quality of life, and lower healthcare costs.
